USE OF PRIVATE INFORMATION:

We value the privacy of each of our customers, affiliates, and associates, and take very seriously, the responsibility to safeguard personal information from abuse. Any personal information provided in the general course of using this site, or in the process of settling any dispute arising from the use of this site, including but not limited to name, address, phone number, e-mail address, or credit card or bank account information will be kept strictly confidential. As such, we will NEVER, under any circumstance, sell, rent, lease, or otherwise distribute personal information in any way unless such distribution or sharing of said information is due to the following reasons:

  • It is required to forward your information to another department of ours or to one of our vendors for the sole purpose of providing service or support and/or delivering product. This includes using information submitted by potential or existing patients in order to facilitate the creation of an appointment, records verification with another medical office, treatment, or other administrative and financial operations related to treatment received or planned.

  • It is requested by our lawyers or a government legal entity for purposes of resolving a dispute, during the course of an audit, and/or assisting with pending litigation, arbitration, or lawsuit.

COOKIES:

At times this site may employ the use of cookies for the sole purpose of returning a visitor to a desired location and/or to set specific user preferences. Despite the hype concerning cookies, their use at this site is in no way used as a spyware tracking mechanism. Our advertisers may employ the use of cookies when visiting our site through their portals and/or links, which are used solely to improve a user's ability to find Retina Specialists of Southern Arizona and tucsonretinaspecialists.com on the internet. At times, photographs may be used and/or linked to that came from copyrighted sources. These sources have provided a license or permission for said photos to be displayed here. In the rare instance that a patient appears in a photo, they have previously signed a photo release before their likeness was posted. All other brands and names used throughout this site are the property of their respective owners.

NOT LEGALLY BINDING

It should be understood that materials and information including but not limited to photographs, explanations of treatment, and research we link, are subject to change and may differ from the actual treatment delivered or currently accepted practices. No information on this site should be construed as completely accurate, and is subject to change at any time. We reserve the right to refuse treatment at any time, and communications resulting from your use of this site shall not construe a commitment to treat, or otherwise create a physician-patient relationship. We refuse to enter into any contracts or agreements of any kind through e-mail or this web-site without a physically written agreement signed in the presence of both parties and/or legal representatives of both parties, or until a patient comes and agrees to our intake paperwork in person.

LICENSES, MEDICAL SPECIFICS

We certify that the physicians listed on this web-site are licensed physicians in the State of Arizona and that they are in good standing with the Arizona Medical Board. While we strive to include a wealth of information related to the Retina discipline, the information found on this site or the sites we link, should not be used as a substitute for medical advice from your personal physician. A response to a message, e-mail, or other contact initiated through this site should not be considered medical advice, unless such advice is given by one of our physicians or staff in response to care you are already receiving from our clinic. All treatment plans, medications, or changes related to diet, exercise, or activity, should be discussed with your Primary Care Physician and specialist directly.

HIPAA

This practice complies with all regulations related to the Health Insurance Portability and Accountability Act (HIPAA) and the handling of Protected Health Information (PHI) and Personally Identifiable Information (PII). We undergo annual audits of our clinic and systems to ensure that data is secure. As such, we will NEVER display sensitive patient information and strongly recommend against sending such information through this site’s contact forms or e-mail.

There is a Secure Upload Form on our Contact Us page that is specifically designed to encrypt sensitive patient information that you need us to have. Please be aware that files must be in an electronic format that is ready to upload, and that our staff will not necessarily be able to provide technical assistance with record conversion or other issues not directly related to the upload. If you have questions about how to use the form, encounter an error, or find that you need to send us an unusually large number of files, please contact our office for assistance.

We also utilize encrypted e-mail to send and receive information between patients and our staff. When receiving an encrypted e-mail for the first time, you may be prompted to setup a login or answer some basic questions about the recipient. It is important to reply directly to the encrypted e-mail from within the Citrix web portal that opens when receiving the secure message. DO NOT just reply to the e-mail that is sending you the notice that you have an encrypted e-mail. If you have questions about how to open or reply to an encrypted e-mail, please contact our office for assistance.

We are not responsible for any breaches related to PHI, PII, or other sensitive information sent to us through this site, its standard contact forms, or via e-mail, unless the Secure Upload Form or Citrix Encrypted E-mail is used!

PCI

This practice complies with all regulations related to the Payment Card Industry Data Security Standard (PCI-DSS). We undergo annual audits of our clinic and systems to ensure that payment card data is not put at unnecessary risk. Patients should not attempt to send us sensitive payment information including, but not limited to bank account or routing numbers, credit card numbers, or PIN numbers through our e-mail or contact forms. Copays will be collected in-person at the time of service. For assistance with payments toward an outstanding balance, please contact our office and ask for the patient advocate. We are not responsible for any payment information sent to us through this site, its contact forms, or via e-mail!
